{"id":609,"date":"2017-02-09T15:30:01","date_gmt":"2017-02-09T21:30:01","guid":{"rendered":"http:\/\/www.wcisec.org\/?page_id=609"},"modified":"2024-04-26T08:25:01","modified_gmt":"2024-04-26T13:25:01","slug":"medical-review","status":"publish","type":"page","link":"https:\/\/www.wcisec.org\/index.php\/medical-review\/","title":{"rendered":"Medical Review Process"},"content":{"rendered":"<h2>Medical Review Process<\/h2>\n<p>The evaluation of a child&#8217;s\u00a0<em>known or unknown<\/em>\u00a0health or medical conditions and the determination of whether these conditions affect the ability to learn.<\/p>\n<ol>\n<li>A WCISEC member district conducts a domain meeting to determine the need for a Medical Review and that they would like the WCISEC Certified School Nurse to complete this review.\n<ul>\n<li>The following questions can be asked to determine if more information is needed:\n<ul>\n<li>Where there complications during pregnancy\/birth?<\/li>\n<li>Has your child had frequent ear infections?<\/li>\n<li>Has your child ever had an injury or accident that required medical attention?<\/li>\n<li>Has your child required medical care from a doctor\/nurse practitioner in the last 12 months?<\/li>\n<li>Does your child take medication?\u00a0 If so, what medication and what dose?<\/li>\n<li>Is there anything in your child&#8217;s health history that we have not covered that you feel would interfere with his\/her ability to learn?<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"2\">\n<li>The WCISEC member district submits the signed domain paperwork to Anita Fulkerson at <a class=\"\" title=\"\" href=\"mailto:AFulkerson@wcisec.org\" target=\"_blank\" rel=\"noopener\">Afulkerson@wcisec.org (opens email window)<\/a>.<\/li>\n<\/ol>\n<ol start=\"3\">\n<li>The forms are processed and forwarded on to the WCISEC Certified School Nurse Andrea Ratermann.<\/li>\n<\/ol>\n<ol start=\"4\">\n<li>The Certified School Nurse contacts the student&#8217;s parents\/guardians and has them fill out and return the following information:<\/li>\n<\/ol>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Authorization to Release\/Obtain Information Form (for each doctor, including specialists)<\/li>\n<li>Student Health History Form<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ol start=\"5\">\n<li>The student may\u00a0be observed by the Certified School Nurse. \u00a0The Certified School Nurse\u00a0will review the student&#8217;s medical and education records.\u00a0 The parent\/guardian will be contacted for a short interview and follow up from the observation\/record review.<\/li>\n<\/ol>\n","protected":false},"excerpt":{"rendered":"<p>Medical Review Process The evaluation of a child&#8217;s\u00a0known or unknown\u00a0health or medical conditions and the determination of whether these conditions affect the ability to learn.
